Prescriptions

If you are receiving regular treatment, repeat prescriptions may be obtained using the right-hand side of your previous prescription form.  Please tick each medication that you require.

These may be ordered

• In person at reception, allowing a minimum of 48 hours for collection
• By post, enclosing a stamped self addressed envelope, allowing 4 days for delivery
• By using the NHS app. Download here – NHS App – NHS (www.nhs.uk)
• By emailing in repeat prescription requests on: prescriptions.btmp@nhs.net

Please note to prevent mistakes we CANNOT accept telephone requests for repeat medication.

The surgery will require you to have a medication review conducted on an annual basis with a Clinical Pharmacist or Pharmacy Technician, please make sure you are up to date with this check.
