Med3 Fit Notes

If you have been ill for more than 7 days in a row and have taken sick leave your employer will request a Med3 Fit note which can be issued via a health care professional.

Please note for an initial Med3 to be issued this will need to be after a GP consultation.

If you have had a Med3 Fit note issued previously and would like this extending or amending this can be actioned via completion of the below link and selecting Fit Note.
